REQUEST FOR ARTWORK PROPOSAL

COMMUNITY HOSPICE BAILEY CENTER FOR CARING

OUTDOOR FOUNTAINS

 

The new 12-bed Bailey Center for Caring  greatly enhances access to care for patients in St. Augustine and surrounding areas. Its location on the campus of Flagler Hospital places it adjacent to the area’s major healthcare provider, making it convenient and accessible for patients, families and physicians, including local doctors who can continue visiting their hospice patients. With this facility, Community Hospice will be able to provide the full continuum of hospice care in St. Augustine.

 

Continuing the Hospice focus on improving quality of life, all elements of the Bailey Center are designed to help families have the best experience possible. The serene and beautiful surroundings are enhanced through art program that features local St. Augustine artists.

 

The Center will open in January 2011.

ART PROJECT

 

TWO LOCATIONS ARE IN NEED OF ARTWORK

·         Front or North Garden

·         Courtyard or South Garden

 

BUDGET           $5,000 FOR EACH FOUNTAIN

 

Requirements:

            Must be a permanent installation able to withstand hurricane force winds, external temperature fluctuations, etc.

            A water reservoir that will be automatically filled (there is water and electric run to each location)

            A water filter to maintain some degree of purity and reduced maintenance

            Artisan/Vendor responsible for installation and will work with contractor on proper foundation for piece

            Artisan/Vendor selected will work with architectural and landscaping designers to discuss proper  lighting for the piece

           

Size & Design Characteristics:

            Front Garden – about 6’ High, 2-3’ Wide – ideally a fountain but could be a sculpture, more artistic in design

Courtyard Garden – Will sit in front of an 8’ wall, ideally about 6’ high, perhaps a half circle/traditional Spanish design with approximately 6’ width from right to left edge, 3’ in front.  Could have sculpture or carving detail.

 

 

Eligibility

This Request for Proposals is open to all.

 

 

Selection Process

An Art Selection Committee may be composed of, but not limited to, administration, foundation, architecture and design team. The panel will review all artists’ submissions.

3. Aesthetic appeal/craftsmanship

5. Functionality and Durability

 

Timeline

Request for Proposals sent out by 12/23

Art submittals due on 1/25/11

Selection panel reviews submittals and makes decision by 2/10/11

Art Installation completed by 5/1/11

Submittal Requirements

All artists and/or teams responding to the RFP must submit complete responses to the information requested in this Section and note any exceptions to any information contained in the RFP. Applicants shall present information in a clear and concise manner following the format below:

 

A. Proposal:

 

·          Illustration/photos: Specify if images proposed are actual pieces for purchase or typical work for artist to be commissioned from.

 

·         Pricing of submitted work or commission schedule along with any discounts offered to Community Hospice

 

B.  Statement of Qualifications

 

·         Resume: a current professional resume no longer than two (2) pages. If submitting as an artist team, a resume should be submitted for each team member, each no longer than two (2) pages. A lead artist should be designated as the primary contact.

 

·         References: Three professional references (particularly if you have completed any projects with similar scope/scale)

 

·          Portfolio: a current portfolio of complete projects which show the range, creative ideas and craftsmanship of your work
